                        **out of state immunization certificates can be converted to KY

                        Immunization Certificates by a physician or the local Health Dept.

Physical Examination Form (within 2 wks. of enrollment)

                        704 KAR 4:020 Section 2.1

                        **physicals for kindergarten enrollees must be no older than 1 yr.

                        of initial entry into kindergarten

                        **Gr. 1-5 enrollees may present a copy of the physical given upon

                        the child’s initial entry into kindergarten, whether in KY or out of state

                        **A second physical is required within one (1) year prior to

                        entry into the 6^th grade

                        **Gr. 7-12 enrollees may present a copy of the physical given at

                        6^th grade, whether in KY or out of state

KY Dental Examination Form (By Jan. 1 of year of enrollment)

**This form must be completed by a Doctor, Registered Physician, dentist, or dental hygienist  for initial enrollment into any KY public school of a 5 or 6 year old child

KY Eye Examination Form  (By Jan. 1 of year of enrollment)

                        704 KAR 4:020 Section 2.6

                        **This form must be completed by an optometrist or ophthalmologist

                        for initial enrollment into any KY public school (this would include

                        first time kindergarten enrollees, as well as any student K-12

                        transferring from a private or home school)

Social Security Card  (within 30 days of enrollment)

                        **a copy of the card must be given to the school.  If longer than 30

                        days, proof of application must be given to the school.  (Copy of the

                        check or money order will suffice as proof.)

Birth Certificate  (within 30 days of enrollment)

                        **a copy of the certified birth certificate must be given to the school

Custody Papers  (upon enrollment)

                        ** “Order of Appointment of Guardianship/Conservator for Minor”

                        from the Court.  (A notarized transfer of guardianship from the parent

                        will not be accepted.)

Most recent report card/transcript (upon enrollment)

                        **at the high school, a copy of the student’s transcript is required

                        **a copy of the most recent report card is not mandatory, but would be

                        helpful for placement of new enrollees at the elementary/intermediate/

                        middle school level
